Request an Appointment

To make an appointment request , please complete our online form or phone the office. Appointment requests can be made by family members, carers, friends, health professionals, support coordinators or funding bodies. A GP referral is not necessary unless you wish to use a Medicare Care Plan to subsidise the costs. Once you have submitted your request online, we will contact you within 2 business days to answer any questions and make a booking.
